Job summary

Gaudenzia is seeking counselors in Harrisburg, PA to manage caseloads, perform assessments, and develop individualized treatment plans in a substance use disorder program. Qualifications

• Bachelor's degree with 1 year clinical experience
• Bachelor's degree with 2 years clinical experience
• Bachelor's degree with 3 years clinical experience; or Master's degree with related field experience

Responsibilities

• Be responsible for a caseload of clients, as identified by program requirements and in accordance with state regulations.
• Complete required evaluations and assessments, including screening, intake, clinical assessments, ASAM and biopsychosocial histories.
• Complete individual treatment plans upon client's admission.
• Document all clinical services and interventions performed within the client's electronic health record and complete monthly treatment reports.
• Maintain accurate electronic health records on each client and ensure timely completion of tasks, within 24 hours of service delivery, no later than 72 hours.
• Conduct individual and group sessions based on client's treatment plan goals and level of care.
• Participate in funding source reviews and make recommendations for treatment and lengths of stay.
• Monitor client progress and adjust treatment plans as needed.
• Coordinate with agency and community resources to ensure a thorough treatment plan.
• Assist clients with developing a discharge/continuing care plan and connecting to resources needed to reach personal goals during and after treatment.
• Follow up with clients after discharge based on regulatory guidelines until established with other case management/social services.

Education

Bachelor's degree with 1 year clinical experience

Bachelor's degree with 2 years clinical experience

Bachelor's degree with 3 years clinical experience; Master’s degree in Social Work, Psychology, Chemical Dependency, Counseling, or Nursing with no experience required; or Master’s in related field with one year clinical experience
