Job summary

Grace Community Care & Homes is developing its Addiction Treatment and Recovery department and seeks an Addiction Treatment Navigator. The role will contribute to person-centered services, reliable operations and accurate documentation.

The navigator will support treatment engagement, coordinate appointments, address barriers, link to resources and follow up within an approved office-based addiction treatment or overdose recovery model.

Qualifications

• Education, professional credentials and experience meeting the OBAT or OORP navigator pathway.
• Eligibility must be verified before enrollment.
• Final regulatory eligibility will be verified before appointment or service delivery.
• Applicants should provide relevant credentials, experience, availability and a resume.

Responsibilities

• Support treatment engagement, appointments, barriers, resource linkage and follow-up within an approved office-based addiction treatment or overdose recovery model.
• Work collaboratively with department leadership, intake, credentialing, billing and quality teams. Maintain confidentiality, respect participant choice, communicate service gaps promptly, and follow role‑specific policies and professional scope.
• Participate in orientation, competency review and quality improvement.
