Job summary

Grace Community Care & Homes is developing its Addiction Treatment and Recovery department and seeks an Addiction Treatment Navigator. The role will contribute to person-centered services, reliable operations and accurate documentation.

The navigator will support treatment engagement, coordinate appointments, address barriers, link to resources and follow up within an approved office-based addiction treatment or overdose recovery model.

Qualifications

• Education, professional credentials and experience meeting the OBAT or OORP navigator pathway.
• Eligibility must be verified before enrollment.
• Final regulatory eligibility will be verified before appointment or service delivery.
• Applicants should provide relevant credentials, experience, availability and a resume.

Support treatment engagement, appointments, barriers, resource linkage and follow-up within an approved office-based addiction treatment or overdose recovery model.

Work collaboratively with department leadership, intake, credentialing, billing and quality teams. Maintain confidentiality, respect participant choice, communicate service gaps promptly, and follow role‑specific policies and professional scope. Participate in orientation, competency review and quality improvement.

Program readiness

This is a planned role. Start date and duties are contingent on applicable program approvals, funding, credentialing and staffing readiness. Services may begin only within the approved scope.

Location and schedule

New Jersey, with administrative coordination through Grace and on‑site or community assignments as applicable. This is a part‑time role. The final assignment and weekly schedule will be discussed during hiring. Clinical services begin only at approved locations and within the program’s authorized scope.

Pay: $22-$28 per hour, based on qualifications and relevant experience.

Employment type: Part-time. Weekly hours and start date will be discussed during hiring; this planned program role is contingent on required approvals and staffing readiness.
