Job summary

Porch Light Health in Boulder, CO is seeking a Certified Physician Assistant or Nurse Practitioner to join our Addiction Medicine team. This full-time role involves clinic-based care with telehealth, treating patients with opioid and other substance use disorders.

Qualifications

Active, unrestricted state license as NP or PA with DEA registration (Board Certified).
Minimum 1 year of clinical experience; at least 0.5 years in addiction medicine/MAT clinics.
In-depth knowledge of MAT protocols and related medications.

Responsibilities

Provide clinical assessment, diagnosis, and ongoing management for MAT patients.
Prescribe and monitor MAT medications, ensuring proper dosing and adherence.
Conduct comprehensive medical histories, exams, and evaluations.
Collaborate with physicians, counselors, and other healthcare providers on treatment plans.
Educate patients about substance use disorders and MAT processes; offer lifestyle counseling.
Monitor progress and adjust treatment plans; coordinate care for comorbidities.
Maintain accurate EHR documentation for encounters and plans.
Ensure compliance with federal/state regulations and MAT guidelines; document controlled substances appropriately.
Provide crisis intervention and de-escalation when needed; ensure safety.
Participate in quality improvement initiatives to enhance care outcomes.
