Job summary

Porch Light Health in Cortez, Colorado, seeks an Addiction Counselor or licensed therapist to provide outpatient group and individual therapy and help expand Medication for Addiction Treatment (MAT) services at our site. This in-person role includes telehealth delivery across multiple sites.

Responsibilities include clinical assessments, individualized treatment planning, evidence-based therapy (Motivational Interviewing, CBT), psychoeducation, and collaboration within a multidisciplinary

Qualifications

• Must hold an independent clinical licensure in Colorado (LCSW, LPC, LMFT, LAC, CAS or equivalent).
• Preferred: CAS or advanced credential; CAT/CAS considered with relevant experience.
• Experience in SUD treatment or behavioral health strongly preferred.
• Strong commitment to harm reduction, trauma-informed, patient-centered approaches.

Responsibilities

• Conduct clinical assessments and develop individualized treatment plans.
• Provide individual and group therapy using evidence-based modalities (e.g., Motivational Interviewing, CBT).
• Deliver psychoeducation and brief interventions to support patient goals.
• Collaborate within an integrated, multidisciplinary medical team.
• Maintain accurate clinical documentation in compliance with standards.
• Participate in case management and referrals as needed.
• Support development and implementation of new programs and services.
• Provide care in-person and via telehealth.
