Zusammenfassung

Porch Light Health seeks an independently licensed therapist to join our Grand Junction, CO clinical team. You will facilitate outpatient therapy, contribute to our virtual group program, and collaborate with medical providers to support MAT services.

The role emphasizes addiction treatment and harm reduction in a fast-paced clinic, with both in-clinic and telehealth delivery across sites in rural areas.

Qualifikationen

• LCSW, LPC, CAS or other independent clinical licensure are required or preferred.
• Clinical experience in substance use disorders (SUD) is preferred but not required.

Aufgaben

• Assess patient needs.
• Coordinate and treatment plan.
• Psychoeducation.
• Provide individual and group therapy.
• Brief interventions - Motivational Interviewing.
• Work in a fast paced medical clinic.
• Work in an integrated and multidisciplinary setting.
• Maintain clinical documentation per regulatory standards.
• Case management and referral services as needed.
• Support implementation of new programs.

Kenntnisse

Independent licensure (LCSW/LPC/CAS)

SUD experience preferred
