Job summary

LiveWell Homecare Agency, LLC is seeking a compassionate Registered Nurse to assess, plan, and deliver patient-centered care in a homecare setting. You will coordinate with the interdisciplinary team, perform treatments and assessments, and ensure safety and quality in daily practice.

Qualifications

• Must hold a nursing license (RN).
• Ability to perform comprehensive assessments and coordinate care.
• Strong communication, problem-solving and critical-thinking skills.
• Commitment to safety, quality and continuous learning.

Responsibilities

• Perform assessments and develop individualized care plans with the team.
• Provide direct patient care, including treatments, medications, and procedures.
• Evaluate intervention effectiveness and adjust plans accordingly.
• Collaborate with physicians and staff to address patient needs.
• Participate in decision-making, quality improvement and safety initiatives.
• Troubleshoot equipment issues and ensure optimal care delivery.
