Job summary

Lifepoint Rehabilitation at Genesis Medical Center - Davenport in Davenport, IA is seeking a dedicated Case Manager to support patients and families, assess needs, and coordinate services.

You will work with the healthcare team to review progress, assist with discharge planning, identify resources, and ensure treatment plans are followed. A social work background and IA licensure are required, with BLS certification encouraged.

Qualifications

• Education: Bachelor's Degree required; Master's preferred.
• License: Current Social Work license for the state of IA.
• Certifications: Basic Life Support (BLS) within 60 days of hire.
• Minimum Work Experience: Minimum 2 years work experience in similar field of social work.

Responsibilities

• Interviews and assesses patients and patient's family, caregivers, and/or legal representatives.
• Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
• Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient’s treatment plan.
• Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
• Collaborates with providers, nursing and ancillary staff.
