Job summary

Lifepoint Rehabilitation is seeking a dedicated Case Manager to join our Davenport, IA team at Genesis Medical Center. You will assess patients and families, coordinate services, participate in case reviews, and assist with discharge planning to ensure smooth transitions of care.

Minimum 2 years in social work, IA Social Work license, and BLS within 60 days are required; Bachelor's degree is required with a preference for a Master's.

Qualifications

• Bachelor's Degree required; Master's Preferred.
• Current Social Work license for the state of IA.
• Basic Life Support (BLS) certification within 60 days of hire.
• Minimum 2 years work experience in a similar field of social work.

Responsibilities

• Interviews and assesses patients and family, caregivers, and/or legal representatives.
• Determines, prioritizes, provides and/or arranges needed internal and external services/interventions.
• Participates in case reviews to evaluate case management and progress; consults with healthcare team to promote, monitor, and evaluate compliance with treatment plan.
• Assists with discharge planning and processes; identifies resources and provides referrals as part of the discharge plan.
• Collaborates with providers, nursing and ancillary staff.

How you'll contribute

A Case Manager (CM) who excels in this role:

• Interviews and assesses patients and patient's family, caregivers, and/or legal representatives.
• Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
• Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
• Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
• Collaborates with providers, nursing and ancillary staff.

What we're looking for
