Job summary

MUSC Community Physicians (MCP), part of the Medical University of South Carolina, seeks a Social Worker (MSW) for PRN discharge planning and care coordination in the inpatient/Institute of Psychiatry setting. You will perform psychosocial assessments, arrange outpatient follow‑up, and support the care team to improve patient outcomes.

Requires a Master's in Social Work, SC LMSW/LISW licensure, and strong communication.

Qualifications

• Master's degree in social work from an accreditation body.
• LMSW/LISW licensure required; LISW-CP preferred.

Responsibilities

• Perform psychosocial assessments for patients and caregivers.
• Coordinate discharge planning to ensure smooth transitions.
• Provide direct clinical social work services, including group therapy and case management.
• Identify post-discharge risks and apply interventions to reduce them.
• Bridge discharge to outpatient providers and address questions.
