Job summary

Carle Health is seeking a licensed Physical Therapist to evaluate and treat patients of all ages, developing and adjusting plans of care to improve function. The role emphasizes education for patients and families, collaboration with the rehab team, and adherence to clinical standards.

The position may include supervision of rehab staff and requires ongoing education to stay current with stroke care guidelines and hospital policy. A sign-on bonus is offered where applicable.

Qualifications

• Bachelor's degree in Physical Therapy required.
• Licensed Physical Therapist (PT) in good standing.
• BLS certification within 30 days of hire per American Heart Association.
• Completion of required continuing education for acute stroke care.

Responsibilities

• Designs and adjusts care plans with goals, outcomes and discharge plans.
• Educates patients, families, and caregivers using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes.
• Performs examinations with reliable, valid methods.
• Document and maintain accurate records per CTSCP67 policy.
• Supervise PTA and rehab technicians as permitted.
• Communicate status, discharge needs, and equipment requirements to the medical team.
