Job summary

The candidate must hold a Physical Therapist license in Illinois, complete BLS within 30 days, and participate in continuing education focusing on acute stroke care. This role involves collaboration with the rehabilitation team and direct patient care.

Qualifications

• Bachelor's Degree in Physical Therapy.
• Licensed Physical Therapist (PT) in Illinois.
• BLS certification within 30 days of hire.
• Attend and satisfactorily complete required CEUs for acute stroke care.

Responsibilities

• Transfers with assistance, practicing safe body mechanics.
• Treat and educate patients and families with empathy and respect.
• Design and adjust plan of care with goals, outcomes, and discharge plan.
• Educate patients, families, and other health professionals.
• Evaluate clinical findings and determine PT diagnoses and outcomes.
• Perform examinations using appropriate methods and document per CTSCP67.
• Screen patients for therapy needs and monitor care by support staff.
• Complete annual education per DNV Hip and Knee Replacement program.
