• Continuing education on acute stroke care as required.

Responsibilities

• Designs an appropriate plan of care that integrates goals, treatment, outcomes and discharge.
• Evaluates clinical findings to determine PT diagnoses and outcomes.
• Educates patients, families and staff using effective teaching methods.
• Performs examinations and documents per CTSCP67 policies.
• Screens need for therapy services and coordinates with support personnel.
