Job summary

Carle Health in Springfield, IL is seeking a licensed Physical Therapist to evaluate and treat patients of all ages, develop and adjust plans of care, and serve as a clinical resource for staff.

The position offers a sign-on bonus and requires active IL PT licensure, BLS certification, and ongoing education in acute stroke care. You will collaborate with the rehab team and help guide patient progress throughout the care pathway.

Qualifications

• Bachelor's degree in Physical Therapy or related field.
• Licensed Physical Therapist (IDFPR) in Illinois.
• BLS certification within 30 days of hire.
• Continuing education in acute stroke care as required.

Responsibilities

• Designs and implements an individualized plan of care with goals and discharge planning.
• Evaluates clinical findings to determine PT diagnoses and outcomes.
• Educates patients and families using effective teaching methods.
• Documents outcomes and follows CTSCP67 documentation standards.
• Supervises PTA and rehabilitation technicians per regulatory standards.
