Job summary

Qualifications

Responsibilities

• Provides direct patient care per established nursing plan and policies.
• Performs nursing assessments and develops an individualized plan of care in collaboration with patient/family and care team.
• Updates care plan based on patient condition and communicates changes to the team.
• Document and coordinate care with multidisciplinary team to ensure safety and quality.
• Participates in performance improvement and adheres to regulatory requirements.
