Job summary

Gundersen Health System is hiring a full-time Social Worker to join the acute care team on the La Crosse main campus. The role involves discharge planning, care coordination, and resources support for acute care patients.

Responsibilities include assessing biopsychosocial needs and providing interventions to support the treatment plan, while acting as a consultant to promote comprehensive care across the organization. Minimum requirements include a Bachelor’s in Social Work and WI license.

Qualifications

• Bachelor’s degree in Social Work or related field required.
• WI Social Worker license required.
• Minimum 1 year experience in social services in hospital, medical clinic, county social service agency or nursing home preferred.

Responsibilities

• Provide discharge planning, care coordination, care navigation, and resource connections for acute care patients.
• Conduct supportive listening and long-term care planning to support individualized treatment plans.
• Assess biopsychosocial needs and deliver interventions aligned with patient care goals.
• Act as consultants and leaders in comprehensive care across the system, supporting organizational strategies.

Education

Bachelor’s degree in Social Work or related field

WI Social Worker license

1 year experience in social services in hospital/clinic/nursing home
