Job summary

Froedtert Health in Milwaukee, WI seeks a Social Worker to coordinate discharge plans for hospitalized patients from admission through discharge, with biopsychosocial assessments and ongoing updates to meet evolving care needs in Care Coordination Rounds seven days a week.

The role requires a Master’s in Social Work and a current Wisconsin social work license, with at least two years in acute care; on-site schedule Monday to Friday with some weekend/holiday coverage.

Qualifications

• Master's degree in Social Work is required.
• Current State of Wisconsin license as a social worker is required.
• Two years of experience in an acute care setting preferred.

Responsibilities

• Coordinate the discharge plan of care from admission through discharge.
• Provide biopsychosocial assessments and timely updates to address evolving needs.
• Collaborate with the multidisciplinary team in Care Coordination Rounds seven days a week.
