Job summary

Gundersen Health System is seeking a full-time Social Worker to join the Adult Inpatient team on the La Crosse main campus. The role offers a 40-hour workweek with Monday–Friday daytime hours, no weekends, and a starting wage of $25.95/hour plus experience-based increases.

You will provide discharge planning, care coordination, and long-term care planning for acute care patients. You will assess biopsychosocial needs, develop interventions, and act as a consultant across the system to support

Qualifications

• Bachelor’s degree in Social Work or related field.
• WI Social Worker license is required.
• 1 year of social services experience in hospital/clinic/nursing home desirable.

Responsibilities

• Provide discharge planning, care coordination and care navigation for acute care patients.
• Assess biopsychosocial needs and support treatment planning.
• Act as consultant and leadership across the system to support goals.
