Job summary

Qualifications

• Education - Associate degree in Nursing; Bachelor’s degree in Nursing preferred.
• Experience - One year experience as a registered nurse.
• In lieu of one year of RN experience, Prisma Health Nurse Residency Program or comparable residency accepted.
• In lieu of one year of RN experience, 1 year of LPN experience for internal candidates within the same service line.
• In lieu of an Associate degree in nursing, a nursing diploma with licensure accepted.

Responsibilities

• Collects data and completes nursing assessment using age-specific standards and tools.
• Develops an individualized plan of care in collaboration with patient, family, and care team.
• Updates the plan as patient condition changes and communicates to the team.
• Identifies measurable goals and discharge plans for continuity of care.
• Provides direct nursing care within the established plan and within RN scope of practice.
• Delegates tasks to team members and ensures completion.
• Monitors quality of care and uses performance indicators for improvement.
• Maintains patient safety and documents care appropriately.
