Kindred Hospital Chicago - Northlake is a 94-bed long-term acute care hospital focused on extended recovery. This role provides planning and delivery of nursing care, coordinating with physicians, family, and interdisciplinary staff to support patient wellness.

Responsibilities include documenting assessments and care plans, participating in discharge planning, and educating patients and families on pain management and post-discharge needs. Strong communication and teamwork are essential.

Qualifications

• Graduation from a nursing program and state licensure.
• Ability to plan, implement and document care using the nursing process.
• Strong communication with patients, families and care teams.

Responsibilities

• Provide direct and indirect patient care through the nursing process.
• Develop and update nursing care plans in coordination with the team.
• Communicate changes in patient condition to physicians and staff.
• Participate in discharge planning and patient education.
