Qualifications

• Current Basic Life Support (BLS) certification.
• BSN preferred for advanced practice and leadership roles.

Responsibilities

• Assesses and coordinates multidisciplinary care across the continuum.
• Facilitates patient communication, advocacy, and education in a culturally competent manner.
• Triages new referrals to ensure timely access to care.
• Conducts post-discharge follow-up and acts as patient liaison navigating care.
