Kindred Hospital Baldwin Park is seeking a Registered Nurse to plan and deliver direct and indirect patient care through the nursing process. You will develop and coordinate care plans with patients, families, and interdisciplinary teams, communicate changes in condition to physicians and Nurse Managers, and participate in discharge planning.

The role requires RN licensure, BCLS, ACLS preferred, with six months of Medical/Surgical nursing experience preferred.

Responsabilidades

• Provide planning and delivery of direct and indirect patient care through the nursing process.
• Develop nursing care plans in coordination with patient, family, and interdisciplinary staff.
• Communicate changes in patient’s clinical condition with physicians and care team.
• Participate in discharge planning process.
• Direct, supervise, and evaluate nursing care provided to patients.
• Document admissions, reassessments, and care plans in the medical record.
• Assess and reassess pain and educate patient/family on pain management.
• Revise the plan of care based on patient response and daily evaluation.
• Receive orders, ensure transcription accuracy, and document completion.
• Administer medications using the five rights and reduce medication errors.
• Assist physicians with examinations, treatments, and procedures within scope of practice.
• Provide teaching to patients and families and involve them in the care plan.
• Prioritize tasks and delegate appropriately to nursing team.
• Interacts professionally with patients, families, and healthcare team.
