Qualifications

Responsibilities

• Collects data and completes nursing assessment using age-specific standards and tools.
• Develops an individual plan of care with patient and family and interdisciplinary team.
• Updates the plan as patient condition changes and communicates with the team.
• Identifies goals and outcomes prior to discharge.
• Provides direct care per established nursing plan and assigns tasks as needed.
• Ensures patient safety and maintains a compliant environment.
• Document progresses and participates in quality improvement initiatives.
