Job summary

Temple Health, a major healthcare provider in Pennsylvania, seeks an RN Case Manager to coordinate patient care across the entire acute episode. You will work with physicians, patients, and families to ensure efficient, cost-effective progress and an appropriate transition plan within 48 hours of admission.

Active PA RN license and BSN are required. One year of acute care RN experience is required, and two years in hospital-based case management is preferred.

Qualifications

• Graduate of an accredited nursing program (RN).
• BSN degree required.
• Minimum 1 year RN experience in acute care.
• 2 years hospital setting experience as Case Manager preferred.
• PA Registered Nurse license required.
• Certified Case Manager preferred.
• Multi-State/Compact RN license acceptable.

Responsibilities

• Facilitate patient progress through acute care episode in a cost-effective manner.
• Monitor level of care and initiate evidence-based protocols and order sets.
• Propose transition plan within 48 hours of admission.
• Advocate for patient and family throughout the acute episode.
