Job summary

Temple-Health is seeking an RN Case Manager for the Jeanes Campus in Philadelphia. This 40-hour day-shift role collaborates with the patient/family and interdisciplinary team to guide the patient through the acute episode of care, ensuring efficient care and safe transitions.

Required BSN; graduate of an accredited nursing program; PA RN license (or multi-state compact) is required; 1 year of acute care RN experience is required, with 2 years hospital case management preferred.

Qualifications

• 1 year RN in acute care required.
• 2 years hospital case management preferred.
• PA RN license required or Multi-State Compact license.

Responsibilities

• Facilitates patient progress through the acute episode of care with the patient/family, physicians and interdisciplinary team.
• Monitors level of care, initiates evidence-based protocols and order sets, and proposes a transition plan within 48 hours of admission.
