Temple Hospital Jeanes Campus in Philadelphia is seeking an RN Case Manager for 40 hours weekly on a day shift. The role collaborates with patients, families, physicians, and the care team to ensure efficient progress through the acute episode and a timely transition plan within 48 hours of admission.

The candidate must be a BSN graduate with at least 1 year of RN experience in acute care; 2 years in a hospital setting as a Case Manager is preferred.

Formación

• 1 year experience as a Registered Nurse in an acute care setting.
• 2 years experience in a Hospital setting as a Case Manager preferred.
• PA Registered Nurse License Required or Multi State Compact Registered Nurse License

Responsabilidades

• In collaboration with the patient/family, physicians, and the interdisciplinary team, the Case Manager facilitates the patient's progress through the acute episode of care in an efficient and cost effective manner.
• Monitors the level of care, initiating evidence based protocols and order sets according to the patients' admission diagnosis, influencing the progression of care, facilitating the patients' navigation through the acute care episode, and proposes an appropriate transition plan within 48 hours of admission.
• The Case Manager serves as an advocate for the patient and family throughout the entire acute episode of care.
