Job summary

LiveWell Homecare Agency in Georgia is seeking a licensed Physical Therapist to provide comprehensive therapy services that improve mobility, reduce pain, and prevent deformities. You will assess, plan, and implement individualized treatment in collaboration with physicians, patients, and families.

You will document evaluations and sessions, maintain confidentiality and ethical standards, and contribute to a positive patient experience through teamwork and continuous education.

Qualifications

• Bachelor’s, Master’s, or Doctorate in Physical Therapy.
• Licenses including Physical Therapist licensure and BLS certification.
• Effective communication and customer service orientation.

Responsibilities

• Assess patients, develop treatment plans, set goals, and discharge strategies.
• Implement therapy plans, educate patients and families, and provide discharge instructions.
• Document evaluations, treatment sessions, and family education accurately per protocols.
• Maintain confidentiality, punctuality, ethical practices, and effective communication.
• Demonstrate respect, foster collaboration, apply service recovery principles, and deliver positive patient experiences.
