Collaboration with the care team ensures smooth discharge planning and patient outcomes. The position requires BLS, PT licensure or eligibility, and a Bachelor's in Physical Therapy, with preferred 1 year of PT

Formación

• Required BLS (Basic Life Support)
• Required PT (Physical Therapy) license or eligible to obtain
• Or PT license temporary with licensure within 6 months
• Required Bachelor's Physical Therapy
• Preferred 1 year Physical Therapist experience

Responsabilidades

• Perform comprehensive physical therapy evaluations for acute-care patients.
• Develop and implement individualized, evidence-based treatment plans.
• Provide interventions including therapeutic exercise, gait training, balance activities, and mobility training.
• Document evaluations, treatments, progress notes, and discharge recommendations.
• Communicate with physicians, nurses, case managers, and other care team members.
