Job summary

PeaceHealth Southwest Medical Center in Vancouver, WA, seeks a Physical Therapist for Acute Therapy as a Per Diem/Relief (0.00 FTE, Variable). The role evaluates patients, creates plans of care, and provides therapy within the PT scope to achieve outcomes with a collaborative care team.

Qualifications

• WA Physical Therapist license required
• Basic Life Support (BLS) required
• Education: Physical Therapy degree from an accredited program (PT)
• Per Diem/Relief position – 0.00 FTE, variable schedule

Responsibilities

• Evaluates patients and establishes a treatment plan with goals
• Delivers treatment per plan and updates as needed
• Completes documentation per regulatory and professional standards
• Collaborates with healthcare professionals to ensure coordinated care
• Mentors and provides oversight to staff and students
• Participates in development of new services and quality improvement
• Attends staff meetings and caregiver orientation
• Performs other duties as assigned
