Job summary

Advocate Health in Milwaukee is seeking a licensed Physical Therapist for full-time Acute Care Rehabilitation. 32 hours weekly with a weekend program offering 8-hour days Thursday through Sunday. Eligible for a weekend differential of $11 per hour on Fri–Sun.

The role focuses on evaluating referrals, planning care, documenting progress, and coordinating discharge, home programs and family education. Requirements include a Doctorate in Physical Therapy and state licensure, with BLS.

Qualifications

• Doctorate degree in Physical Therapy from an accredited school.
• Licensed Physical Therapist in the state of practice.
• BLS for Healthcare Providers within 90 days of hire.
• No experience required; CS/IT skills helpful.
• Demonstrated organization, decision-making and problem-solving abilities.

Responsibilities

• Interpret referrals and perform musculoskeletal, neuromuscular, cardiopulmonary and/or integumentary evaluations.
• Develop and implement a plan of care with regular outcome evaluations.
• Provide discharge planning, home programs and family teaching as needed.
• Document all patient care, progress notes and discharge summaries per policy.
• Supervise staff and maintain productivity and safety standards.
