Advocate Health Sherman Hospital is seeking a licensed Physical Therapist for our Acute Care Rehabilitation department in Elgin, IL. The role is full-time with 40 hours per week, including 2 evenings and one weekend every four weeks.

Prior acute care and EPIC EMR experience are preferred; strong patient-centered care is essential. Responsibilities include evaluating and treating patients, planning discharges, and coordinating with physicians and staff to ensure high-quality outcomes.

Formación

• Doctorate degree in Physical Therapy from an accredited program.
• Current/licensed Physical Therapist in the state of practice.
• Experience in acute care is required; ED experience a plus.

Responsabilidades

• Interpret physician referrals and perform musculoskeletal, neuromuscular, and cardiopulmonary evaluations.
• Develop and implement individualized treatment plans and monitor outcomes.
• Complete discharge planning with home programs and family education.
