Jackson Purchase Medical Center in Mayfield, KY is seeking a Pharmacist to join our team. You will provide pharmaceutical care, review orders, educate staff and patients, and support quality and regulatory standards.

Applicants must hold a Doctor of Pharmacy degree and a current license; BCLS is required and ACLS is preferred. You should thrive in a fast-paced environment and be proficient with pharmacy software systems.

Qualifications

• Doctor of Pharmacy degree with active licensure in the state.
• BCLS required; ACLS preferred.
• Ability to work with minimal supervision in stressful environments.
• Proficiency navigating pharmacy/clinically oriented software systems.

Responsibilities

• Coordinates and provides pharmaceutical care for patients within the interprofessional team.
• Reviews and verifies orders, supports drug therapy protocols and education.
• Manages drug therapy protocols as part of an interdisciplinary team.
• Interprets medication orders, prevents drug-related problems, and resolves issues.
• Provides drug information to staff, professionals, patients, and students.
• Documents interventions and outcomes for quality improvement.
• Verifies orders before entry; ensures accuracy of medications prepared by staff.
• Manages anticoagulation clinic patients, including dosing and monitoring.
