Qualifications

• Experience as a CNA in an acute/hospital setting.
• Able to read, write at a level to fulfill documentation requirements.
• Physical demands: standing, walking, lifting up to 100 lbs., etc.
• Ability to follow care plans and maintain patient dignity and privacy.

Responsibilities

• Provides all ADL cares to patients including oral, grooming, hygiene, dressing, bathing, ROM, ambulation, dining.
• Delivers care according to patient care plans.
• Communicates changes in patient condition to staff nurse promptly.
• Maintains cleanliness of equipment and adheres to safety protocols.
• Records entries on daily flow sheets per policies and procedures.
• Interacts with interdisciplinary team to establish individualized care plans.
