Holy Cross Hospital in Fort Lauderdale, FL is seeking an Emergency Department RN Case Manager to coordinate patient care from the ED, assess admission necessity, and ensure safe discharge transitions.

This role works with physicians, insurers, and families to remove barriers to care and arrange post-acute services such as home health, SNF, equipment, or follow-up visits. Florida RN license and BLS required; ACLS preferred.

Qualifications

• Graduation from an accredited school of Professional Nursing is required.
• Current licensure in the State of Florida is required.
• BLS from AHA; ACLS preferred.

Purpose

An emergency department (ED) RN case manager coordinates patient care, evaluates admission necessity, and manages safe discharge transitions directly from the emergency room.

What You Will Do

• Screens incoming ED patients to see if they meet acute care criteria.
• Arranges post-acute care services, such as home health care, skilled nursing facilities, medical equipment, or outpatient follow-up appointments.
• Communicates with insurance companies, physicians, and families to remove barriers to care and reduce re‑admissions

Minimum Qualifications

• Graduation from an accredited school of Professional Nursing is required.
• Prior Utilization Review experience is required
• Excellent clinical assessment skills as well as good verbal and written communication skills are necessary.

Licensure

• Current licensure in the State of Florida
• BLS Required from AHA; ACLS Preferred
