Job summary

Holy Cross Hospital in Fort Lauderdale, Florida is seeking an experienced ED RN Case Manager to coordinate patient care from the emergency department, evaluate admission needs, and manage safe discharge transitions.

You will screen ED patients for acute care criteria, arrange post-acute services (home health, SNF, equipment), and communicate with insurers, physicians, and families to remove barriers and reduce readmissions.

Qualifications

• RN must be licensed in Florida (FL)
• Prior Utilization Review experience required
• Excellent clinical assessment and communication skills required

Purpose

What You Will Do

• Screens incoming ED patients to see if they meet acute care criteria.
• Arranges post-acute care services, such as home health care, skilled nursing facilities, medical equipment, or outpatient follow-up appointments.
• Communicates with insurance companies, physicians, and families to remove barriers to care and reduce re‑admissions

Minimum Qualifications

• Graduation from an accredited school of Professional Nursing is required.
• Prior Utilization Review experience is required
• Excellent clinical assessment skills as well as good verbal and written communication skills are necessary.

Licensure

• Current licensure in the State of Florida
• BLS Required from AHA; ACLS Preferred
