Qualifications

• Masters in Social Work from an accredited Institution.
• At least 3 years of relevant social work experience.
• LCSW licensure required; ACM, CCM preferred.

Responsibilities

• Prioritize referrals from interdisciplinary team members, patients, and families.
• Coordinate discharge planning and safe transitions to appropriate levels of care.
• Educate patients and families about discharge planning and community resources.
• Maintain documentation per hospital policy and ensure timely chart updates.
