Live Well Homecare Agency, LLC in Pennsylvania seeks a Pharmacist to provide safe, effective medication therapy and develop evidence-based care plans focusing on patient outcomes and quality of life.

Responsibilities include reviewing orders, assessing drug safety, collaborating with medical teams, supervising staff, preparing sterile medications, and participating in rounds to optimize pharmacotherapy for homebound patients in our clinical program.

Qualifications

• Bachelor's Degree in Pharmacy required; Doctoral Degree preferred.
• Strong pharmacotherapy knowledge and excellent communication.
• Teamwork and supervision skills; able to manage complex medication regimes in a clinical setting.

Role: The Pharmacist is responsible for providing safe, effective, and cost-efficient medication therapy, developing and monitoring evidence-based pharmacotherapy care plans focused on patient outcomes and quality of life.

Key Duties:

• Deliver pharmaceutical care tailored to specific patient care areas.
• Interpret, clarify, and review physicians' medication orders.
• Assess medication profiles for safety, effectiveness, and interactions.
• Recommend formulary alternatives and advise medical staff on drug therapy issues.
• Monitor laboratory results and patient parameters; provide drug information and discharge consultations.
• Collaborate with medical teams, participate in rounds, and perform pharmacokinetic dosage calculations.
• Prepare compounded sterile medications and manage pharmacy supplies.
• Supervise pharmacy technicians, students, and interns, ensuring accuracy and adherence to standards.
• Coordinate with healthcare professionals to resolve pharmacy-related issues and assist in policy updates.

Qualifications & Skills:

Bachelor's Degree in Pharmacy (required), Doctoral Degree preferred. Strong knowledge of pharmacotherapy, excellent communication, teamwork, and supervision skills. Ability to work in a clinical environment and handle complex medication management.
