Job summary

• Pharmacist degree with active licensure or eligibility in the US.
• Ability to collaborate with multidisciplinary healthcare teams.
• Strong clinical and pharmacotherapy knowledge.

Responsibilities

• Assumes responsibility for delivery of pharmaceutical care in the assigned patient care area.
• Reviews physician medication orders for safety and appropriateness.
• Consults with medical staff on drug therapy problems and monitors lab results.
• Provides drug information to healthcare professionals and patients.
• Assists with discharge medication consultations and coordinating care.
• Prepares medications requiring compounding and sterile techniques.
• Supervises pharmacy staff and ensures adherence to standards.
