Job summary

Geisinger is seeking a Pharmacist to provide quality pharmaceutical care, ensuring safe, effective, and cost-conscious medication therapy for all patients. The pharmacist will develop, monitor, and adjust evidence-based pharmacotherapy plans with emphasis on patient outcomes and quality of life in a clinical setting.

Responsibilities include reviewing orders for accuracy, counseling patients, coordinating with physicians and care teams, and participating in rounds.

Qualifications

• Bachelor's Degree in Pharmacy required.

Responsibilities

• Assumes responsibility for the delivery of pharmaceutical care to the specific patient care area in which the pharmacist is scheduled to work.
• Coordinates resources to ensure timely, efficient, cost effective and appropriate care.
• Reviews patient medication profiles for indications, effectiveness, safety and adherence issues.
• Reviews all medication orders for prescribing, formulary and therapeutic issues.
• Consults with medical staff on drug therapy problems and monitors patient lab results.
• Provides drug information to medical staff and other healthcare professionals.
• Provides patient discharge medication consultations based on department standards.
• Makes rounds with medical services.
• Calculates drug dosages based on pharmacokinetic parameters.
• Supervises and trains pharmacy technicians, students, and interns.
