• Identifies patients with complex psychosocial needs and refers to Clinical Social Work for follow up.

Responsibilities

• Oversees notification for Medicare rights per CMS regulations, including related notices.
• Ensures collaboration of the healthcare team and communicates during rounds.
• Directs referral management tasks for discharge transition with the transportation coordinator.
• Provides comprehensive EMR documentation and continuity of care information.
• Routinely identifies avoidable days/variances and documents them prior to discharge.
