Qualifications

• Basic Life Support Certification (BLS) for healthcare providers.

Responsibilities

• Assess physical, social, and psychological status; gather information from the care team.
• Develop an individualized plan of care with patient and family involvement.
• Direct interdisciplinary care team to deliver care per plan and coordinate tasks.
• Evaluate patient outcomes and revise the care plan as needed.
• Maintain professional standards, documentation, and regulatory compliance.
