Job summary

Cascadia Healthcare in Medford, OR is seeking an experienced Activity Director to design and implement resident-centered activity programs across a diverse population.

You will conduct comprehensive assessments, develop individualized activity plans, coordinate staff and budgets, document outcomes, and collaborate with therapy, social services and nursing to enhance quality of life and maintain regulatory compliance.

Qualifications

• High school diploma with two years of social or recreational programming experience in the last five years.
• One year in a patient care setting required.
• Certified Activity Director (NCCAP or state-specific) certification.
• Valid driver’s license.

Responsibilities

• Perform comprehensive assessments to determine abilities and interests.
• Develop individualized activity programs tailored to residents.
• Complete MDS, CAAs timely and accurately.
• Document resident responses and evaluate plan effectiveness.
• Coordinate off-site activity trips when appropriate.
• Collaborate with interdisciplinary team to promote outcomes.
• Organize activity calendars and communications for residents and staff.
• Supervise Activity Assistants and ensure timely attendance.
