Job summary

Rise, Inc. in Minnesota seeks an Activity Lead/Coordinator to develop and deliver skill-building, life-enrichment activities for adults with disabilities in community and center-based programs. You will coordinate transportation using Rise buses/vans while ensuring person-centered supports.

You will work with Rise Supervisor and Case Managers to align activities with program plans and build community partnerships for volunteers. The role emphasizes service excellence and mobility assistance.

Qualifications

• Proficient with Microsoft Office (Outlook, Word, Teams).
• Learn Rise case management software.
• Familiarity with AAC and assistive technology systems.
• Valid driver’s license and clean driving record.
• Compliance with MN DHS Rule 11 and Rise background checks.

Responsibilities

• Create, schedule and provide skill-building, life enrichment and recreational activities for adults with disabilities.
• Provide transportation services using Rise buses/vans.
• Coordinate calendars and maintain activity supplies.
• Collaborate with Rise Supervisor and Case Managers to keep programs person-centered.
• Develop partnerships to create volunteer opportunities.
• Support individuals with a person-centered approach and assist with mobilities as needed.
