You will lead and implement an engaging activity program to enrich residents' physical, mental, and social well-being, interviewing residents and families to tailor plans, coordinating with staff, and documenting progress.

Qualifications

• Experience in long-term care, home & community care, or similar settings.
• Strong communication and interpersonal skills.
• Background in event planning or recreational therapy.
• Familiarity with dementia and Alzheimer's care best practices.
• Understanding of social work principles and person-centered care.
• Creativity, flexibility, and a collaborative spirit.

Responsibilities

• Plan and execute a wide range of engaging, inclusive activities
• Interview residents and families to create personalized activity plans
• Maintain accurate documentation of services and progress notes
• Coordinate with other departments and provide transportation for outings
• Supervise activity staff and volunteers, and assist residents with crafts, projects, and appointments
• Promote family involvement and foster a warm, supportive environment
