Job summary

Yoakum Nursing and Rehabilitation Center is seeking an Activity Director to plan and lead the resident activity program and maintain essential documentation. You will organize both individual and group activities, ensure coverage for all residents, and develop care plans with timely MDS sections.

The role involves coordinating off-site activities, engaging with the community, recruiting volunteers, and contributing to QA processes. Strong organizational and communication skills are essential.

Qualifications

• Must have a high school diploma or equivalent.
• Bachelor’s degree with 1 year of full-time experience working with elderly or disabled in human services.
• 60 semester hours with 2 years of full-time experience working with elderly or disabled in human services.
• Completed an activities director's course, and two years of full-time experience working with elderly or disabled in human services.

Responsibilities

• Organize both individual and group activities for residents.
• Ensure multiple activities occur for high and low functioning residents.
• Complete section E of the MDS timely and accurately.
• Develop the activities component of the Comprehensive Care Plan.
• Complete annual activity assessments for residents.
• Provide activities for bedfast residents (one-to-one) and document accordingly.
• Prepare monthly calendar and post for residents and visitors; share with residents.
• Coordinate minimum two off-site activities per month.
• Participate in community events related to facility interests.
• Recruit, orient, and supervise volunteer program.
• Participate in QA meetings and complete Monthly QA reports.

Primary ResponsibilitiesThe Activity Director will be responsible for planning, coordinating, and directing the resident’s activity program and the maintenance of necessary documentation.Essential FunctionsOrganize both individual and group activities based on the needs of the residentsEnsure that multiple activities are occurring for both high and low functioning residentsComplete section E of the Minimum Data Set (MDS) timely and accuratelyDevelop the activities component of the Comprehensive Care Plan from the completed activity assessmentComplete an annual assessment for each resident on the activities component of the Resident AssessmentProvide activities for residents that are bedfast and/or unable to participate in group activities (one to one) and documents in the appropriate recordPrepare monthly calendar and post in an area that is accessible and highly visible and frequented by residents and visitors. Calendar should also be provided to each resident and posted in roomPrepare the facility newsletterCoordinate a minimum of two off-site activities per monthParticipate in community events related to the interest of the facility and arrange for use of community facilitiesCollaborates with Corporate Events/Activities Coordinator in organizing various employee and non-employee events for the facility.Recruits, orients, and supervises the volunteer program of the facilityParticipate in facility QA meetings and complete Monthly QA reportsEducational/Training RequirementsA.
