Job summary

City View Post Acute is seeking an Activity Assistant to join our team. The role supports the Activity Director in planning and delivering a dynamic program for residents, aligning with individual interests and capabilities.

You will help execute monthly calendars, track attendance, and provide engaging materials, including Braille/audiobooks when needed. You will encourage participation, coordinate outings, arrange transportation, and assist with assessments and progress notes.

Responsibilities

  • Participate in planning and conducting activities for individuals, small groups, and large groups.
  • Help create a welcoming environment to encourage participation and social interaction.
  • Assist with monthly activity calendar and attendance records.
  • Encourage self-directed activities (hobbies, crafts, reading, games, music).
  • Provide activity materials, including Braille or audiobooks when needed.
  • Assist with developing activity care plans based on assessments.
  • Assist with discharge planning and resident assessments as assigned.
  • Complete assessment documentation and quarterly progress notes as directed.
  • Coordinate and participate in resident outings and community activities.
  • Arrange transportation for residents for outings, appointments, and discharge.
  • Prepare in-room activities for bed-bound or isolated residents and ensure engagement.
  • Communicate effectively with residents, families, coworkers, and community members.
  • Assist QA committee with plans of action for quality concerns.
  • Maintain a clean, organized, safe Activity Department; perform other duties as assigned.

Activity Assistant – Full TimeCity View Post Acute is looking for a compassionate, energetic, and creative Activity Assistant to join our team! If you enjoy working with seniors, creating meaningful experiences, and bringing joy and engagement to residents’ daily lives, we’d love to hear from you.Position: Activity AssistantSchedule: Full Time – Day ShiftHours: 9:00 AM–5:00 PM or 10:00 AM–6:00 PMPay: $18–$22 per hour, DOE (Depending on Experience)General PurposeThe primary purpose of the Activity Assistant is to assist the Activity Director in planning and conducting a creative, interactive, and meaningful activities program for our residents.The Activity Assistant helps ensure that activities are designed to meet the individual interests, abilities, and physical, mental, and psychosocial needs of each resident.
