Job summary

Prairie Manor Nursing and Rehab in Chicago Heights, IL is seeking an empathetic Activity Aide to enhance daily living for residents through engaging activities and social interaction. You will support individualized plans and help create a warm, vibrant community.

The role involves facilitating group and individual activities, documenting progress, and coordinating events while following facility policies. Prior experience in healthcare activity programs is preferred.

Qualifications

• 1-2 years in a social or recreational program in a healthcare setting (preferred).

Responsibilities

• Facilitate ongoing group and individual activity programs based on residents’ needs, interests, and abilities, as identified in assessments and care plans.
• Complete required record-keeping upon resident admission, discharge, and transfer; chart and document per facility policy.
• Assist in creating the activity calendar and coordinating events.
• Follow and maintain established policies and procedures.
• Encourage resident participation in Resident Council and schedule monthly meetings.
