Valir PACE seeks a dedicated team member to support participant engagement under the PACE Program Director. You will assist in intake, assessments, care planning, and committee work, ensuring activities align with regulations and participant needs.

The role emphasizes accurate documentation, calendar creation, and maintaining a safe, well-equipped environment while collaborating with a diverse team and participants. CPR, and relevant education, are required.

Qualifications

• Associates or Bachelors in a relevant field (arts, education, recreational therapy).
• Licensed occupational therapist or OT assistant or OSHD training.
• Experience in recreational activities or long-term care environments.
• Strong communication and organizational abilities are essential.

Responsibilities

• Assist in intake screens, care planning, and committee activities as directed.
• Complete comprehensive assessments for new enrollees and periodic renewals.
• Create monthly activity calendars and notes for participants' charts.
• Maintain accurate patient records and ensure a safe, well-stocked environment.
