Job summary

Valir PACE in Shawnee, OK is seeking a dedicated Activities Director to support participant engagement and therapeutic recreation within ourPACE program. The role involves coordinating intake, assessments, and ongoing care planning, while leading group and individual activities and maintaining accurate records.

The ideal candidate will have CPR certification and a relevant degree, plus experience in recreation or long-term care settings, with a focus on compassionate patient-centered care.

Qualifications

• Current CPR certification (required)
• Associates or Bachelors degree as listed
• Licensed OT/OTA or OSOHD approved course
• 1 year experience in recreation or long-term care and enrollment in OSHD approved Activities Director course

Responsibilities

• Participate in intake screens, IDT, care planning and PAC committees
• Complete comprehensive assessments for new enrollees and renew every 6 months
• Create monthly activity calendars and center activity plans
• Document activity notes and patient records
• Maintain safe and clean environment with stocked supplies and functioning equipment
• Familiar with care plans and adhere to policies to ensure proper care
• Report changes observed in condition or behavior
• Other duties as assigned

Qualifications:

Job Summary: Under the supervision and/or direction of the PACE Program Director, performs various duties assisting in the activities and engagement of participants in accordance with the established methods and procedures of Valir PACE.

Duties/Responsibilities:

• Participate in intake screens, IDT, care planning, PAC committee and other committees as directed by the PACE program director
• Complete comprehensive assessments for every new enrollee, renew assessments every 6 months and reassess as needed for changes of status
• Creates activity plan for center, ensuring appropriate hours of activities are scheduled and carried out in accordance with regulations.
• Creates monthly activities notes for each patients chart
• Create activity calendar each month
• Create ADC slideshow and renew every month
• Facilitate and run group and individual activities.
• Report and/or records any changes observed in condition or behavior.
• Accurate and timely documentation.
• Assist in maintaining a safe and clean environment with well-stocked supplies and functioning equipment.
• Familiar with duties and performs them accurately, recognizing their importance in relationship to patient care.
• Familiar with and adheres to care plan, observes established policy and procedures to assure proper treatment and care of participants.
• Maintains accurate and timely patient records.
• Other duties as assigned.

Education, Licenses, Certifications and Experience

• Current CPR certification (required)
• Associates degree or baccalaureate from an accredited university or college in art, music, physical educations, recreational therapy, education, or similar program,
• Or; Licensed occupational therapist or occupational therapy assistant,
• Or; Successful completion of an OSDH approved training course,
• Or; One year experience in a recreational activity or long term care environment, and is enrolled within 180 days of employment, in an OSDH approved course for activities directors.
