Job summary

Apploi is seeking an Activities Assistant to support daily recreation programs for residents, under the Activity Director. You will help plan and implement activities on Sundays, Mondays, and Fridays, fostering a warm, patient environment and collaborating with a team.

The role emphasizes resident-centered care, documentation, and coordination with families as appropriate. Part-time, per diem position with comprehensive benefits for eligible staff.

Responsibilities

• Assists in planning and implementation of organized program for recreation.
• Provides a planned resident-specific recreation program; facilitates small group activities as assigned by the director.
• Develops a written plan of care as part of the resident's recreation program and follows it daily.
• Involve residents and families in assessment and care planning when possible.
• Transport residents to and from recreation programs.

Essential Job Functions:

• Assists in planning and implementation of organized program for recreation


• Provides a planned resident specific, goal directed recreation program for assigned residents/patients; facilitates small group activities as assigned by the director


• Ensure that all charted recreation progress notes are informative and descriptive of the services provided and indicate the residents/patient's response to the service. Assists in developing a written comprehensive plan of care for each resident/patient recreation program that identifies the problems/needs of the resident/patient and the goals to be accomplished for each problem/need. Follow the established plan of care in providing the daily recreation program.


• Involve the resident/patient and family members in the assessment and care planning process when possible.


• Transport residents/patients to and from recreation programs.
